Troubleshooting Automation Error: Notes and Record ID Required

Hello need help with this automation: I try to write notes after a called is completed, but I get an error message: " I notes and record id required " when refreshing the output to test. and on an actual call the automation is working from When a new contact is created on GHL , Filtered lead by lead type, then Call Contact, then send SMS with call status to myself. up to that point it works as a charm. but the notes is not being written. at one point this week i got it to work, but i don't know what happens.
